Hi!
You will have a great time on the Disney cruise! You may never see your little one because they love the Disney kids programs! Key West is really fabulous - I did the little trolley tour that hits the best spots in Key West, including going past Hemingway's mansion, seeing the Key West architecture, the southernmost point of the US. It is a great tour and you get a really good glimpse of the town - they will let you off in town if you want to shop or wander the streets. On the pier, check out the shark feeding that goes on, kids and grown-ups find it really interesting. There is lots of scuba diving in Grand Cayman and Cozumel and you might also consider splurging on swimming with dolphins. Castaway Cay is just the BEST place - the best thing to do is just hang out on the beach and play in the water for the day. It is so gorgeous, the tropical fish will swim right up and around your legs.
Have a great cruise!

Not sure what to do with a little one in Key West--maybe the Conch train tour, or just find a nice beach. We enjoyed the tour of Hemingway's house (a neat place with lots of cool cats) but it might be a little slow for a four-year-old.
In GC, definitely do a stingray tour--Capt. Marvins or Capt. Bryan are both good, but I've heard even better things about Nativeway lately. The stingrays swim around above a shallow sandbar where you'll be able to stand, so you can make sure your child is in good hands. (I would recommend booking this directly--you'll save $20-$30 per person).
In Cozumel, Chankanaab Park has dolphin encounters as well as other stuff for kids. If you want to do dolphins, reserve in advance--otherwise you can just take a cab from the ship.
Enjoy!
