Jamaica!
Rafting on a bamboo raft down the Martha Brae.
Bicycle tour through the Blue Mountains.
Zipline canopy tour.
Live reggae in Negril.
Bob Marley/ Peter Tosh "reggae history" tour.
This is just the tip of the iceberg of things to see and do on this amazing island. I know the people on this board who have been many times can give you a list as long as your arm of things to see and do.
You could go 100 times and never experience all Jamaica has to offer.

Barbados has just about everything the islands have to offer, except for gambling and a volcano. See www.barbados.org

We just returned from our second trip to Jamaica (the first was 9 years ago) and I must second Diana's recommendation.
We have visited several other islands - and we're quite fond of the US and British Virgins - but the interior of Jamaica is hard to match. In addition to the ideas mentioned above, beautiful waterfalls at Mayfield, YS, and Dunn's River; boat safaris on the Black River; Appleton Rum Estates tour.
The lush, hilly interior of the island was the prettiest country I've seen since visiting East Africa some years ago.
I have also heard good things about Dominica.

In my own I would recommend Jamaica above all other islands I've visited. Previous posters have already enumerated its many charms and activities available.
From what I've read, Puerto Rico also offers what you're looking for. From the architectural, historical center in Old San Juan to the big city life of San Juan, to the only rainforest in the US in El Yunque, to the pristine beaches of Culebra and Vieques, to the bioluminescent bays in Fajardo and Vieques, to the surfing capital in Rincon, PR has a LOT to offer. I've got my first trip planned there in May.
Don't know where you're from, but if you're not a US citizen, I would suggest checking out Cuba as well.

As allready mentioned .... Jamaica , Cuba ( if possible ) Dominica , St.Vincent and the Grenadines , Tobago ..... Mountains , rainforests , waterfalls , hiking , beaches , snorkelling , diving , incredible vistas , great " irie " factor , etc. etc.

I agree with Diana also about Jamaica. What a place! So much to see and do. The island is quite large. No matter how many times I will never see it all. Want to hit the south area near Treasure Beach and visit that Pelican Bar one day.
