I have done this many times. Here's what you do.
You should have a letter confirming your reservation at the Casa Particular (legal private home). A fax copy is sufficient.
Put the house address on your tourist card. The letter is merely so the Cuban Immigration officer knows that you are in a legal home.
Another alternative (which I also have done) is simply to put the name of a Cuban hotel in the city you are staying at (I use the National in Havana) on the tourist card. There is no check by the Cuban authorities of the validity of what you write down. However remember that they might ask you (althought I have never been asked) for your letter of confirmation.
Your choice
Steve
When I applied for a tourist card to visit Cuba and had the name of a legal homestay on the application the Cuban Embassy in Sydney actually changed it and put down a hotel. So I suggest you put down a hotel and avoid any problems.
If you are bringing expensive gifts to host, then I would put down the name of the hotel, otherwise putting down the "legal private home" address is not a problem.
