Luxury golf vacation in November

My husband and I are planning on taking short anniversary trip/babymoon this November (2014) and I'm hoping all the expert travelers in the peanut gallery might chime in and help us decide on a destination. We are coming from DC and our two priorities are golf (for my husband) and low key luxury resort on a sandy beach (for me). Also, because I'm pregnant, availability/accessibility of decent medical care for an emergency is a concern, as is food/water safety. Adults only isn't necessary, but it would be nice to have a mostly kid-free trip. Activity wise, other than golf, a spa would be nice, but I'm mostly a beach lounger on vacation; and we enjoy good restaurants. Budget wise, we'd like to keep it around $5k for accommodations (not including airfare or food, if not an AI). Past vacations we've enjoyed were Belize/Guatemala (the three Coppola resorts); Anguilla -- the best!! (Cuisinart); Grand Cayman (condo w/baby) -- we found food on the island terrible; and Playa Mujeres (El Dorado Maroma) - nice but food was so-so.

We considered the Bahamas' Sandals Emerald Bay, but I'm not sure the huge AI experience is us (in the past we've enjoyed quieter resorts) plus I've seen a lot of terrible reviews and reports of a lot of sand flies/bugs. We almost pulled the trigger on Tortuga Bay in Punta Cana, but I've heard some OB's aren't comfortable with emergency care in DR. The Bucuti in Aruba looked perfect but unfortunately we're late to the game and they're booked. We are now looking at Barbados; Nevis; Puerto Rico (St. Regis Bahia, but it doesn't really excite me); or maybe curaçao. We haven't really considered any of these islands before, so if you have any insight or suggestions of others we should be looking at, would love to hear it! Thanks!
