Looking for recommendations for a short family trip to Puerto Rico

We only have about 5 days for a summer vacation this summer. Leaving on July 4th and returning on July 8th. We're a family of four with an 11 yr old and almost 13 yr old. We like to go to different places and seem to focus on the Caribbean and Central America, with a couple of cruises in the mix. But with 5 days, we can't venture too far. I've looked at Florida as well, but Puerto Rico is now catching my eye in my initial research, but I don't know enough about it to make a decision yet. Between our travel time, we really would only have 3 full days. We'd be flying out of Baltimore or Philly. Baltimore is usually less expensive.

My husband and I spent a day in Old San Juan on a cruise stop and loved it. That's about all I know about Puerto Rico!

I know we would want a resort type hotel, with nice pools and on a swimmable beach. We would like to get to the rain forest. Although we have spent time in Costa Rica - would this be much of a different type experience? I would also like to see the bioluminescent bay. I think the kids would enjoy seeing the forts as well. Our budget would be as close to $200 - $250/night. Ideally a hotel that we could get a rollaway bed would be great, since I have a boy and girl and they can't stand sharing a bed at their ages! Or 2 double beds with a sofa bed.

I have looked at El Conquistador because of the waterpark, but I'm not sure about being so isolated. I don't think we'd want to eat every meal at the hotel, so I think we'd have to rent a car and that's another additional expense. For a 5 day vacation, I really am not looking to break the bank.

So, any hotel and itinerary help you could give me would be wonderful!
