Petit St. Vincent in the Grenadines is an all-inclusive (except for alcoholic beverages.) Palm Island, also in the Grenadines, is nice (a notch lower than PSV, IMHO) and includes everything, even alcoholic beverages, but, oddly, not bottled water. Curtain Bluff in Antigua is a solid AI as well, but I prefer the Grenadines personally.

We've have been to 14 AI hotels in Mexico/Caribbean.
Mexico, Jamaica and the Dominican Republic with have the vast majority of AI hotels. You will need to be much more specific in your critieria, who is going and budget in order to recommend a resort in one of those locations.
As for the rest of the Caribbean, here a list of some hotels that offer an AI or an AI type plan: (this list does not inlcude AI resorts that are adult only)
Aruba - Aruba Marriott Rst & Stellaris Casino, Holiday Inn Sunspree Resort Aruba, Occidental Grand Aruba, Riu Palace, Westin
Antigua - Hawksbill by rex resorts, Jolly Beach Resort and Spa, St. James's Club, The Verandah Resort & Spa
Curacao - Breezes and the Marriott
Grand Cayman - The Reef Resort and the Marriott
Nassau - Breezes, RIU and the Paradise Island Harbor Resort
St. Kitts - The Royal St. Kitts and the Marriott
St. Lucia - Almond Smugglers Cove, Coconut Bay Beach Resort & Spa, St. Lucian by Rex Resorts, Windjammer Landing Villa Beach Resort
St. Martin - Sonesta Great Bay Beach Resort, Sonesta Maho Beach Resort & Casino, Le Flamboyant Hotel & Resort
St. Thomas - Bolongo Bay Beach Resort, Marriott Frenchmans Reef&Morning Star, Wyndham Sugar Bay Resort & Spa
Grenada - Grenadian by Rex Resorts
Depending on where you are coming from and how long you are wanting to stay will be a factor also as some Islands are much harder to get to than others.
If you can narrow down excactly what you are looking for, we can better provide you with recommendations.
