Sightseeing in Kathmadu Valley

How much should I expect to pay for a two half-days guided tour (including private car and driver) to see Baktapur, Patan City, Pashupatinath, and Swayambhunath in the Kathmandu Valley? How much is an appropriate tip for driver and guide?
