How to get from Kyoto to Hokkaido by JR rail?

Does anyone have any information on how to get from Kyoto to Hokkaido by JR rail? (e.g. do I have to change from one railway line to another railway line at which station?) I will be purchasing a JR pass.

Any info is much appreciated.
