Day tours in Kuala Lumpur

I'm trying to help a friend plan a day long layover in KL between flights from Perth to the US. She'll arrive at 8 am and depart at 7 pm. She's looking for a tour of the main sights, including Batu Caves.

I suggested a tour from the airport as it seems there a few options, but she's not had any response to her e-mails, and at least one of the sites appears to be under construction. It's been awhile since we were last in KL, so my info is dated.

Does anyone happen to know if tours from the airport must be booked in advance, or can one just turn up?

I believe it's also possible to book a taxi for a day and work out an itinerary with the taxi driver...has anyone done this? Any idea of cost and how to go about making arrangements, or can it be done on the fly?

Any suggestions appreciated.
