There are lots of guides and drivers in Bali. We use a husband and wife team called Made and Connie. Email [email protected] We were very happy with their service, care and attention last time we were in Bali and will use them again at the end of this month. They organized our white water rafting at a greatly reduced price from what was advertised. It was fantastic but be ready for the stairs down to the river and at the end back up the ravine. We choose not to do elephant riding as we had done that in Thailand but I am sure she could arrange that and anything else you might decide that you want to do. I am sure you will have a great time in Bali.

For the rafting, I've done it twice with Sobek, and love them. Equipment and guides are excellent, as are the facilities at both ends.
For elephants, there is just one camp near Ubud, so not much choice. This is not a very big thing in Bali.

I second the recommendation for Sobek rafting. There are others but Sobek is one of the oldest and most reliable. You don't need a guide. Just have your resort call Sobek and make an appointment. They will pick you up and take you back to wherever you are.
Elephants are not native to Bali and always seem out of place to me.
