New to Dividend Miles- how does it work?

My family of four may be flying PHL-HNL on US Airways this summer, and I've decided that if we do, I should definitely join Dividend Miles to get the 10,000+ miles per person that the flight is.

We have 2 adults and 2 children. Can I just create one account for myself, and use that dividend miles number for all four of us, or do we have to create 4 separate accounts?

If I get the dividend miles credit card, can my wife also get one, so that we earn 50,000 bonus miles between the two of us?

If we have to make 4 separate dividend miles accounts, is it possible to combine the miles from the four accounts so that we have enough miles for one or two free tickets on a later flight, or not?

Lastly, does anyone know if when booking a vacation through US Airways, do you earn any extra dividend miles for booking the entire thing through them (flight, hotel, car), or just the miles you earn for the flight?

Sorry about this question. I know its very basic, but i can't find any definitive answers on usairways.com. Any help is greatly appreciatd. Thanks!
