Travelling with my elderly Mum to Jordan, Israel and PA

I will be travelling with my elderly Mum who can't walk fast and has many ailments such as diabetes, wears a cochlear implant and suffers from sleep apnea. It will be my first time travelling with my Mum to a far off destination, and I am a little scared that I will not be able to cope or cater for her specific needs. Or even worse.... lose patience!
Can anyone shed any light on a similar situation where they traveled to the middle east with their parent/parents and how you managed? Also what type of itinerary did you book etc? Any advice would be really appreciated.
